What to Bring
- Insurance cards, driver's license or picture identification
- Any co-pay required by your insurance coverage plan.
- Laboratory results, EKGs, x-ray reports already performed
- The completed Patient Assessment Database.
- The patient's Advance Directive (if one has been initiated). We must have a copy of this for our records for anyone having surgery.
Financial Arrangements
All financial processes should be completed at or before the time of admission, including insurance information, required forms and signatures, assignment of benefits, and payment of any required deposits. A business office representative is available to assist you during your stay with any questions you may have.
